The key locates the flywheel in the proper relationship to the crankshaft and hence timing cannot get drastically out of spec unless someone has messed with the advance linkage. The screw is below the one that is the handle. The one with the handle is for idle adjustment. No you can do this with the prop on. You won't have to go up to full throttle about 2500-3000 rpms.
